Enrolment

We are currently accepting new patient enrolments. To enrol, you can either visit The Practice to pick up a physical enrolment form or download the PDF file below.

After completing the form, please return a physical copy to our reception team at The Practice on Francis Street.

Identity Checking

To confirm your identity, we ask that you provide a piece of photographic I.D. along with your completed enrolment form. This could be a driver’s license, passport, KiwiAccess card, or any other official document which bears your image.

Eligibility

If you are not a New Zealand citizen, we ask that you provide proof of eligibility with your enrolment form. This includes attaching a copy of a valid work visa, residency visa, or any relevant documentation to confirm your status. Without this proof, we will be unable to process your enrolment.
